Immunizations Throughout the Lifespan

From infants to senior citizens, immunizations are one of the most important ways to protect yourself, your family, and those around you. Use the following recommendations to keep you and your family up to date with their immunizations. Check with your health care provider or call 800-275-0659 if you have any questions.

Kindergarten Immunization Requirements

The following vaccines are required to attend Kindergarten in Utah:

  • 5 DTaP/DT (4 doses of DTaP, if 4th dose is given on/after the 4th birthday)
  • 4 Polio (3 doses, if 3rd dose was given on/after the 4th birthday)
  • 2 Measles, Mumps, Rubella (MMR)
  • 3 Hepatitis B
  • 2 Hepatitis A
  • 2 Varicella (chickenpox)
  • 3-4 Hib (Haemophilus Influenza type B)
  • 4 Pneumococcal conjugate (Prevnar)
  • 2-3 Rotavirus (not required)

Seventh Grade Immunization Requirements

The following vaccines are required for students entering seventh grade:

  • 1 Tdap
  • 3 Hepatitis B
  • 2 Varicella (chickenpox)
  • 1 Meningococcal

The HPV vaccine is recommended for pre-teen boys and girls 11-13 years of age.

Annual Influenza vaccine is recommended for both children and adolescents.

Adult Immunization Requirements

  • 1 Td/Tdap, then Td booster every 10 years (Tetanus, Diphtheria, Pertussis [Whooping Cough])
  • 2 Measles, Mumps, Rubella (persons born on or after 1957)
  • 2-3 Hepatitis B
  • 2 Hepatitis A
  • 2 Varicella (chickenpox)
  • 2 Shingles
  • Influenza (yearly)
